Do you ACCEPT iNSURANCE?

I am in-network with BCBS IL PPO plans. I am considered out of network with all other insurance carriers, but can provide a superbill upon request for you to submit to own your own to try and get reimbursement.

Do I need a doctor’s referral?

Maybe, but probably not. As of January 2024 occupational therapist gained direct access in the state of IL allowing for treatment without a referral. Private-pay clients do not need a referral and most commercial insurances are not requiring a referral; however, if one is needed for your specific insurance plan I will work with you to obtain it from your doctor prior to the evaluation.

How do I schedule an appointment?

Where do visits take place?

If you are in Chicago or close surrounding areas, chances are I will come to you!

If you are slightly out of my normal driving rage we can work together to establish a mix of in-person and telehealth treatment.

If you are out of my driving range and still wanting to see me, I have office space in Roscoe Village.

If you are in IL but out of either of our driving ranges we can do telehealth only.
